Patient Complaints & Grievances

We want to hear from you if any part of your experience falls short. Every complaint is reviewed personally by our provider, and no patient will ever be penalized, discharged, or treated differently for raising a concern.
1. How to Submit a Complaint

Please include your name, the best way to reach you, the date of the visit or transaction involved, and a description of the concern. Do not include additional protected health information in an unsecured email — we will move the conversation to a secure channel.
2. Our Response Timeline
- We acknowledge every complaint within two business days.
- We complete our review and provide a written response within 30 calendar days.
- If the review requires more time, we will tell you why and give you a revised date.
- Complaints and their resolutions are documented and retained for quality review.
3. Billing and Refund Concerns
Billing disputes are handled under our Refund Policy, Cancellation Policy, and Membership Refund Policy. Contact us before initiating a chargeback so we can resolve the issue directly.
4. Privacy Complaints
Concerns about the handling of your health information are addressed under our HIPAA Notice of Privacy Practices, which also explains how to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.

This process is not for urgent clinical matters. If you are experiencing a medical emergency, call 911 immediately.
